How do you get this Black and White sort of smoothed out Effect for a photo?

What kind of program/website/effect makes this? Thank so much! 
I'd do it with Lucisart SE or Noiseware. They both do a great job.

It does a really nice job of blending and softening without losing detail in the eyes.
